Support Coordination
Our Support Coordinators provide you with Level 1-3 support. We work based on your plan goals and needs, ensuring that all the support and services are connected and implemented to achieve your goals. Our support Coordinators will guide you all the way through the journey, from preparing your access application, plan meeting, and how to manage your plan review and ensure your plan reflects your evolving and ongoing needs.
Behavioural Support
Our Behavioural Support practitioners conduct Behavioural Support Assessments, provide Behavioural Support Plans, review restrictive practices and provide face-to-face individualised strategies for both our participants, staff and families.
Home Modification
Our team of Complex Home Modification certified OTs work closely with our experienced builders and ensure that your home modification outcomes are above and beyond an a game changer in significantly improving your functional independence.
Community Nursing Care/ Complex High Intensity Care
Our nursing team comprised of Nurse Practitioners, Registered and Enrolled Nurses provides both medical and nursing care to our participants. Our services range from basic nursing care tasks to complex care needs such as PEG, nasogastric tube feeding, ventilator, tracheotomy and urinary catheter management and sub-cutaneous injections.
Personal Mobility Equipment
We pride ourselves in our ability to provide a great quality range of mobility equipment including but not limited to wheelchairs, toileting aids, seat wheel-walkers to aid in your day-to-day activities.
